Flower Show Registration Form

If you wish to have your Flower Show evaluated, please complete the following form and submit to CGCI Flower Show Awards Chairman at least three weeks before the show.  The form will be automatically sent to the FS Awards Chairman. 

  • After receiving this information, the CGCI Flower Show Awards Chairman will appoint the evaluating panel of three judges and notify them that they will evaluate your show.   Your Show Contact Person will be given the names of the evaluating judges.
  • When the panel of three return their evaluation forms to the CGCI Flower Show Awards Chairman, the CGCI Flower Show Awards Chairman will notify your Show Contact Person whether your club is eligible for an NGC or CGCI Award and send the appropriate application form for your Book of Evidence to you.  The due date for the Book of Evidence for any award is December 1.

Name of Show Chairman, Judges Chairman or other who is the contact between FS Awards Chairman and the club.
Include PO Box or Street address, City and Zip Code
List of judges: Include Name, Status (Accredited, Life, Master), Mailing address, Phone number and email (if available).
